Wage: $41.42 - $52.00 per hour


Summary:
The Homecare Nurse (HCN) at Quatsino First Nation assesses, plans, delivers, and evaluates care needs for acute, chronic, and palliative clients of all ages in community settings. Emphasizing independence and health promotion, the HCN provides education, counseling, and guidance to clients, families, and caregivers. This role operates autonomously while fostering interdisciplinary collaboration to ensure excellence in health services and education.

Nursing Practice:
Assess physical, emotional, psychosocial, spiritual, cognitive care needs of clients.

Develop holistic care plans based on client goals for independence and self-care.

Administer direct care, evaluate progress, and revise care plans as necessary.

Provide specialized care in areas such as footcare, diabetic care and education, wound management, pain management, and medication management.

Conduct home visits and utilize support staff for home visit checks as needed.

Conduct comprehensive assessments of clients' living environments to evaluate suitability for care delivery.

Client Advocacy and Education:
Act as an advocate, respecting client choices and supporting caregivers.

Provide guidance, education, and counseling to clients, families, and other caregivers.

Utilize wellness promotion and self-management principles, behavioral change strategies, and adult learning theories to support optimal health.

Educate on medication management, wound care, pain management, and diabetic care.

Conduct educational programs on diet, healthy eating, and diabetic-friendly meal planning.

Make recommendations and provide guidance on home modifications and equipment needs to optimize safety and accessibility for clients receiving home care services.

Collaborate with clients, families, and relevant stakeholders to implement recommended changes and ensure compliance with safety standards.

Collaboration and Community Engagement:
Work collaboratively with clients, families, caregivers, healthcare professionals, and community resources.

Coordinate care issues, cultural barriers, and ethical dilemmas in a culturally sensitive manner.

Conduct case conferencing and discharge planning to enhance client care transitions.

Administrative Duties:
Maintain accurate records and documentation according to protocols.

Prepare statistical data, reports, and correspondence as required.

Participate in interdisciplinary case conferences and team meetings to enhance care coordination.

Provide guidance and training to support staff in home support skills and client care.

Qualifications & Experience
Requirements:
Education and Experience:

Current registration as an RN or LPN with the British Columbia College of Nursing Professionals (BCCNP).

Current CPR certification.

Minimum 2 years of recent experience in community health or acute medical/surgical care.

Skills and Abilities:
Knowledge of chronic disease management, geriatric care, and palliative nursing.

Strong clinical assessment and care planning skills.

Effective communication, facilitation, and conflict resolution abilities.

Proficiency in using electronic medical records and other computer applications.

Commitment to professional development and continuous learning.
