Details:
Working closely with the Health Services Manager, Home Care, Drug and Health Care Workers, the Community Nurse is responsible for providing nursing services to members within the Nation.

Nursing Services
• Apply and utilizes key nursing knowledge and critical thinking to assess, plan, implement, and evaluate interventions to address health issues.
• Document assessments, care plans, reassessments, and service delivery on clients’ personal Home Care Health Records.
• Coordinate nursing care services with the client, physician, hospital, rehabilitation, long term care programs and other community programs.
• Develop care plans, and assist clients to manage their own care and navigate through the various services available within the community setting.
• Develop nursing care and teaching strategies to assist clients/families dealing with chronic nillness.
• Support Adult Activity program initiatives and outings that promote independent living, traditional wellness and continued socialization in and out of the community.
• Establish therapeutic relationships with clients/residents and families and provides emotional support through identifying individual and family perspectives on health, conducting learning assessment and understanding life views and priorities for the individual and family.
Home Care
• Provide individualized care to clients/families by initiating discussions to identify what is important from the perspectives of the client/family, assessing client health status, identifying priorities and plans and discussing professional care and services that support these plans.
• Coordinate home care services in a way that meets the needs of the clients while promoting independence and family community support.
Qualifications & Experience:
• Registered Nurse in good standing with the College of BC Nursing Professionals (BCCNP).
• Current Basic Life Support, Level “C” certificate.
• Valid Class 5 Drivers License and own vehicle.
• Completion of immunization certification and certified within 1 year preferred.
• Completion of additional education in wound and foot care preferred.
• 1-2 years recent experience in public health and home care nursing.
• First Nations experience preferred.
