Details:
The Campbell River Hospice Society provides compassionate end-of-life and grief support to individuals of all ages through the support of trained volunteers and generous donors.
POSITION:
This position is 2 days per week. Wednesdays and Thursday 8 am-4 pm. The Client Coordinator is responsible for managing the needs of all the CRHS’s clients while maintaining quality of service. The Client Coordinator will manage all client intake. As the Client Coordinator, you will report to the Executive Director.
Qualifications & Experience:
Bachelor’s Degree in Social Work or an undergraduate degree; with a minimum of two year’s grief and loss counselling experience. Experience may be considered in combination with relevant education.
Membership in a Professional Counselling Association.
Demonstrated knowledge of the grief process.
Demonstrated knowledge of family systems.
Demonstrated experience with anticipatory grief, compounded grief, complicated grief and traumatic loss.
Knowledge and understanding of the philosophy of hospice palliative care and the needs of the dying, their families and the bereaved.
KNOWLEDGE AND ABILITIES
Ability to provide a high level of care and compassion.
Ability to maintain a client-driven database.
Determine the need for services with the patient/family/caregiver and entire hospice team through an intake process.
Participate as a member of the Interdisciplinary Team and attend other meetings as appropriate.
Ability to communicate with staff, clients, volunteers and community members effectively and professionally, including excellent written, verbal and computer communication skills (MS Office, Excel, Power Point, Access, etc.).
Ability to work efficiently both independently and as part of a team in a fast-paced environment;
Ability to organize and coordinate clients and their needs with strong attention to detail.
Able to ensure accuracy, confidentiality and safekeeping of CRHS’s records.
Maintain communication with the bereaved family following the death of the individual hospice patient.
Ability to maintain appropriate boundaries in the face of sorrow, pain and suffering.
Coordinates the delivery of services (individual, groups and ongoing follow-up) to ensure timely and appropriate services are provided to the client and family/caregivers.
Maintain an ongoing check-in process with all clients accessing service.
Ensure the quality of services by evaluating services and data regularly.
Ability to provide presentations on behalf of CRHS.
Able to provide counselling sessions when needed.

Flexibility to perform other duties as assigned by the Executive Director
